Community Scheduler Role
A core pillar of our firsthand care model is Continuity of Care, meant to help connect the individuals we serve to community providers who will build long-term relationships and deliver high quality care. The Community Scheduler role is key to the success of this process which ultimately improves the quality of life for our individuals! Focused on the coordination and scheduling of appointments with local clinics, the Community Scheduler will own all of the day to day responsibilities of ensuring our individuals are successfully connected to care. The Scheduler collaborates with community team members across multiple locations in different markets and outside organizations to ensure appointments are timed, coordinated, and sequenced appropriately and all associated activities are complete. This role serves as the face of firsthand to clinics across the country! As the Community Scheduler, you will:

  • Serve as a single point of contact for monitoring, managing, and coordinating scheduling/rescheduling of community appointments, including primary care, behavioral health care, and specialty care, upon request from Health Guides or other team members
  • Provides individuals, caregivers, and community team members with information and updates as needed
  • Maintain awareness of multiple competing situations needing attention and prioritize to assure individual needs are met, while maintaining a pleasant, friendly demeanor.
  • Deliver a consistently high level of service to internal and external customers, and go above and beyond to assist individuals in successfully completing community appointments
  • Provide individual / community team members with correct information related to their scheduled appointments in response to any associated questions they might have.
  • Review chart to piece together important information for the patient, provider clinic, or community team member.
  • Identify potential issues that could complicate care coordination (i.e. insurance issues or other concerns)
  • Confirmation of appointment attendance and capturing an individual’s experience rating
You will be a good fit if you have:
  • Desire to help people in need and support their path to wellness
  • Comfort working independently as well as with interdisciplinary teams
  • Excellent customer service, time management, prioritization, and patient communication skills
  • Familiarity with electronic health record systems and basic health insurance knowledge
  • Experience using different tools to communicate with individuals (text, phone, etc) or an affinity for learning new technology
  • Ability to multitask and enjoy a fast paced work environment
  • Attention to detail and thorough documentation
  • Flexibility and persistence working with individuals who may be difficult to reach or need support rescheduling appointmentsInterest in helping shape this role as the company grows
The required experience you bring to this role includes:
  • High school diploma or GED or equivalent
  • A minimum of two years’ experience, in a healthcare setting/practice scheduling appointments across multiple practice sites
This is a full-time, exempt, salaried position reporting to firsthand's Senior Director, Clinical Value. firsthand cares deeply about ensuring pay equity with every team member who joins. Commensurate on candidate experience, the expected range for this role is $35,000 - $40,000. firsthand Benefits:
